Retinovaný semenník u dospelého muža pred plánovanou herniotómiou – kazuistika

Polohové anomálie semenníkov (kryptorchizmus) je patologický stav, znamenajúci neprítomnosť semenníka v skróte. Zahrnuje nezostúpené, ektopické, alebo chýbajúce testes. Ide o pomerne častú vývojovú chybu, postihujúcu 0,8 – 1,8 % chlapcov starších ako jeden rok. Dlhodobé abnormálne uloženie semenníka vedie k zníženiu fertility a k zvýšenému riziku vzniku zhubného nádoru. Keďže v 6. mesiaci dochádza k znižovaniu počtu zárodočných buniek a spontánny zostup po tomto období je vzácny, liečba kryptorchizmu už môže začať v tomto období, mala by byť ukončená do veku 18 mesiacov a zaručiť ortotopnú polohu oboch gonád do 2. roku veku. Základom liečby je včasná diagnostika. V tejto práci uvádzame kazuistiku predoperačne zisteného retinovaného semenníka pred plánovanou herniotómiou u dospelého pacienta.

Retained testicle in adult before a planned herniotomy – a case report
Cryptorchidism is a patological condition in which the testicle is stored outside of the scrotum. The incidence after the first year of age is 0.8-1.8%. Long term abnormal deposit of the testicle in adulthood leads to a reduction in fertility and an increased risk of developing cancer. As in the 6th month there starts a decrease of germ cell count and the spontaneous descent after this age is rare, the therapy of cryptorchidism should start already at this age and should be completed until the age of 18 months. The testis should be brought to its normal position in the scrotum before the child is two year old. Early diagnostics is the main principle of treatment. We present a case of retained testis in patient undergoing herniotomy.
