Myoplastika musculus sartorius v cievnej chirurgii

Infekcia cievnej rekonštrukcie je závažná komplikácia v cievnej chirurgii. Napriek pokrokom v liečbe je spojená s vysokou morbiditou a mortalitou. Jednou z možností liečby je myoplastika pomocou musculus sartorius. Najčastejšie používaným svalom pri riešení infekcií v trigonum femorale je m. sartorius. K jeho výhodám patrí dostupnosť v operačnom poli, jednoduchý a rýchly odber. Je vhodný na prekrytie malých a stredne veľkých defektov, riešenie infekcií Samson III, IV, lymfatických komplikácií a aj ako adjuvantný profylaktický výkon. Výsledky sú výborné pri riešení včasných infekcií, ale pri riešení neskorých infekcií je nutné myoplastiku doplniť o resekciu protézy a jej náhradu žilou. Myoplastika m. sartorius je bezpečný výkon, ktorý má pri dodržaní indikácií dobré výsledky. Vzhľadom na svoju jednoduchosť by mohol rozšíriť spektrum operačných výkonov cievnych chirurgov.

Sartorioplasty in vascular surgery

Infection of vascular reconstruction is a serious complication in vascular surgery. It is associated with high morbidity and mortality despite advances in management. One of the treatment options is myoplasty with musculus sartorius. It is often used for dealing wih the infection in trigonum femorale. Its advantages include the presence in surgical wound, simple and fast harvesting. Sartorius muscle is suitable for coverage of small and medium-sized defects, dealing with infection Samson III-IV and lymphatic complications. It can be also usefull tool as an adjuvant prophylactic procedure. The results are excellent when dealing with early infections. In late infections myoplasty must be accompanied by a vascular prosthesis explantation and its replacement with autologous vein grafts. Sartorius myoplasty is a safe procedure that has good results if the indications are followed. Due to its simplicity it could expand the spectrum of surgical procedures of a vascular surgeon.
