Jak pacientka a neurologové dokázali zvítězit nad chronickou formou cluster headache – případ z naší praxe
V tomto článku je prezentován případ s chronickou formou cluster headache z naší praxe. Jedná se o vzácné onemocnění. Jde o pacientku ženského pohlaví s tímto těžkým neurologickým onemocněním. Obecně byly zmíněny obtížnosti při porozumění patogeneze cluster headache. Naše pacientka během dlouhého sledování vyzkoušela různé druhy klasické medikace, ale žádný lék nevedl k dostačujícímu efektu a kvalita jejího života byla během dlouhé doby dost nízká. U ní ani léčba lithiem, jakožto preventivním způsobem léčby cluster headache, nepomohla. Později byl nasazen galcanezumab s. c., což pacientce přineslo výrazné snížení frekvence a intenzity bolesti hlavy. Naše pacientka trpěla mimo jiné i projevy deprese, které výrazně regredovaly, když se symptomy cluster headache zmírnily. V daném okamžiku se pak dokázala po delší době pracovní neschopnosti vrátit na své pracovní místo, a to dokonce na plný úvazek. K dispozici jsou různé vědecké publikace, které ukazují na pozitivní efekt galcanezumabu u pacientů s cluster headache, avšak většina z nich byla uskutečněna u lidí s epizodickou formou. V tomto textu jsou uplatněna konkrétní vědecká data z různých studií týkajících se galcanezumabu s touto indikací.

How the patient and her neurologists managet to win the fight against the chronic form of cluster headache – a case report
This case report describes a rare disease – the chronic form of cluster headache. Here a female patient with this severe neurological disorder is discussed. The difficulties in understanding the pathogenesis of cluster headache in general are noted. Our patient tried different types of “classical” treatment but none of those led to a sufficient effect and her quality of life was very low for a long period of time. Lithium therapy also did not help. Later, galcanezumab was prescribed in our outpatient clinic, which led to a significant decrease in headache frequency and headache intensity. Our patient also suffered from symptoms of depression, which significantly improved at the time when the cluster headache manifestations improved. Thus, at some point, she returned to her full-time job after a very long period of disability. There are different publications which show the positive effect of galcanezumab in patients with cluster headaches. However, most of the studies were conducted on people with the episodic form. In this text concrete scientific data from different studies about galcanezumab with this indication are also discussed.
