Nepriaznivý zdravotný stav ako súčasť indikácie sociálnej služby

Z dôvodu zhoršovania zdravotného stavu pacienta môže vzniknúť nepriaznivá sociálna situácia, v ktorej je jedným z riešení spolupráca lekára so sociálnym odborom obce a rodinou pacienta. Včasnou intervenciou zo strany lekára sa zvyšuje možnosť pacienta zotrvať čo najdlhšie v domácom prostredí. Výsledkom spolupráce je zabezpečenie posúdenia odkázanosti na sociálnu službu a poskytovanie vhodného typu sociálnych služieb.

Unfavourable health conditions as part of the indication of social services

Because of the deterioration of the health of a patient, unfavourable social circumstances may arise, one of the solutions of which is the cooperation between the physician, the municipal social services department and the family of the patient. An early intervention from the side of the physician increases the chances of the patient to stay in his home environment for as long as possible. The result of this cooperation is the provision of the appraisal of the dependency on social services and of an appropriate type of social service.
