Afázie a možnosti jejího skríninkového stanovení pomocí Mississippi Aphasia Screening Test – české verze (MASTcz)
Jazykové poruchy v rámci afázie, alexie a agrafie jsou rozmanité. Jejich diagnostika, zvláště v časné fázi po cévní mozkové příhodě, je mnohdy komplikovaná především změnami klinického obrazu a koexistujícími poruchami dalších kognitivních funkcí. Cílem orientačního klinického vyšetření pacienta se suspektním narušením fatických funkcí je detekovat symptomy postižení a potvrdit přítomnost afázie s alexií a agrafií. Vhodným nástrojem je standardizovaný skríninkový test MASTcz, který má vysokou senzitivitu i specificitu při detekci fatických poruch, současně orientačně stanoví tíži postižení pomocí jednoduché kvantifikace a zkušený specialista může s jeho využitím určit i typ afázie. Test, splňující nároky kladené na skríninkový test, má své možnosti a limity při klinické aplikaci, které v práci diskutujeme.

Aphasia and potential of screening evaluation using Mississippi Aphasia Screening Test – Czech version (MASTcz)
Language disorders and their demonstrations such as aphasia, alexia and agraphia vary substantially. Their diagnostics, and especially early stages of the disease after stroke, is complicated due to changes in symptomatology and coexisting cognitive disturbances. Clinical examination of a patient with suspected language disorders focuses on detection of aphasia, alexia and agraphia. MASTcz is a standardized screening test with a high sensitivity and specificity in detection of aphasia. It can to a certain extent quantify the severity of language impairment, which helps an experienced aphasiologist to determine particular type of aphasia. The MASTcz test complies with requirements on screening instruments and its limitations are discussed further in the text.
