Hemoroidy a možnosti ich liečby pomocou HAL-RAR

Postupné presadzovanie sa jednodňovej chirurgie a s ňou spojenej ambulantnej anestézie vyplýva aj z možností miniinvazívnych chirurgických výkonov. Tieto operácie majú veľký benefit pre pacienta, čo sa týka ich rýchlejšej rekonvalescencie a menšej bolestivosti, ako aj rýchlejšieho zaradenia sa do bežného života. Metóda HAL-RAR (Haemorrhoidal Artery Ligation – Recto Anal Repair) pri operácii hemoroidov, predstavená už v roku 1995 v Japonsku dr. Morinaga patrí medzi tieto miniinvazívne procedúry, ktoré spĺňajú vyššie uvedené požiadavky. V našej štúdii na 262 pacientoch sme skúmali efekt spinálnej a celkovej anestézie so sledovaním BIS počas tohto výkonu. Zamerali sme sa predovšetkým na potrebu pooperačnej analgézie a možnosť prepustenia pacienta do domácej starostlivosti po tomto výkone so spomenutým typom anestézie. Potreba pooperačnej analgézie bola nutná pri celkovej anestézii po 278,25 minútach po skončení výkonu a pri spinálnej anestézii po 325,04 minútach po podaní anestetika intratekálne.

Haemorrhoids and possibilities of treatment using HAL-RAR
The gradual advancement of one-day surgery and conjoint ambulatory anaesthesia results from possibilities to execute mini-invasive surgeries. Such surgeries offer great benefits to the patient due to faster reconvalescence and less pain as well as faster return to normal life. The HAL-RAR method for haemorrhoid surgery, introduced as early as in 1995 by Dr. Morinaga in Japan, is one of such mini-invasive procedures meeting the above mentioned criteria. The goal of our study including 262 patients was research of effects of the spinal and general anaesthesia while monitoring BIS during the surgery. We focused mainly on the need of postoperative analgesia and possibility to discharge patient to home care after the surgery with the above mentioned type of anaesthesia. The need for postoperative analgesia was necessary for the period of 278.25 minutes after the completion of surgery for the general anaesthesia and 325.04 minutes for the spinal anaesthesia after the administration of anaesthetics intrathecally.
