Anestézia u detí s osteogenesis imperfecta: klinické špecifiká a štandardné odporúčania

Osteogenesis imperfecta zahŕňa heterogénnu skupinu genetických ochorení spojivového tkaniva charakterizovanú zvýšenou krehkosťou kostí a sklonom k fraktúram rôznej závažnosti. Ťažšie formy ochorenia sú často sprevádzané postihnutím respiračného a kardiovaskulárneho systému, čo významne zvyšuje riziko komplikácií v perioperačnom období. V článku analyzujeme špecifiká anestéziologického manažmentu u detských pacientov s osteogenesis imperfecta a uvádzame kľúčové faktory ovplyvňujúce bezpečný priebeh anestézie. Dôraz sa kladie na dôkladné predoperačné vyšetrenie, šetrnú manipuláciu s pacientom pri polohovaní, zaistení dýchacích ciest a zabezpečení venózneho vstupu, adekvátny výber anestéziologickej techniky, zhodnotenie rizika fraktúr a individuálne prispôsobenie anestéziologického postupu. Článok poskytuje prehľad aktuálnych poznatkov a klinické skúsenosti, ktoré môžu prispieť k optimalizácii perioperačnej starostlivosti o túto vysokorizikovú skupinu pacientov.

Anesthesia in children with osteogenesis imperfecta
Osteogenesis imperfecta is a heterogeneous group of inherited connective tissue disorders characterized by increased bone fragility and a high incidence of fractures. In its more severe forms, respiratory and cardiovascular complications significantly increase the perioperative risk. This article reviews the specific anesthetic considerations in children with osteogenesis imperfecta and identifies key factors influencing the safe administration of anesthesia. It focuses on comprehensive preoperative assessment, careful patient handling during positioning and airway management, challenges in establishing vascular access, appropriate selection of anesthetic techniques, fracture risk evaluation, and individualized anesthetic planning. Drawing on the current literature and clinical experience, this review provides recommendations to optimize perioperative care in this vulnerable pediatric population.
