Budúcnosť hypolipidemickej liečby

Súčasné stratégie na prevenciu aterotrombotických komplikácií sú zamerané na modifikáciu rizikových faktorov aterotrombózy prostredníctvom liekov podporovaných evidence based medicine (aspirín/a alebo klopidogrel, β-blokátory, ACE-inhibítory a statíny). Napriek intenzívnemu manažmentu konvenčných rizikových faktorov aterosklerózy kardiovaskulárne reziduálne riziko ostáva stále vysoké. Toto všetko vedie k ďalšej snahe ovplyvniť prognózu vysokorizikových pacientov vývojom nových hypolipidemík, ktoré môžu byť zamerané na ovplyvnenie aterogénnych lipoproteínov (inhibítory squlén syntázy, MTP inhibítory, anti-sense oligonukleotidy pre apo B), na ovplyvnenie metabolizmu antiaterogénnych HDL častíc (CETP inhibítory, agonisty LX receptora, agonisty PPAR-α, agonisty PPAR-β/δ, inhibítory endotelovej lipázy, apo A-I mimetické peptidy), ako aj na vývoj nových liekov redukujúcich subklinický vaskulárny zápal, upravujúcich stabilizáciu vulnerabilného plátu (inhibítory fosfolipázy A2).

Future of hypolipidemic therapy

Strategies for preventing complications of atherosclerotic vascular disease have emphasized risk factor modification using treatment approaches supported by evidence based medicine (aspirin and/or clopidogrel, β-adrenergic blockers, angiotensin-converting enzyme inhibitors and statins). Despite the intensive management of the conventional risk factors is residual cardiovascular risk remaining high. There is the prospect that novel therapies target the atherogenic lipoproteins (squalene synthase inhibitors, MTP inhibitors, antisense oligonucleotides to apo B), target the high density lipoproteins metabolism (CETP inhibitors, LX receptor agonists, PPAR α agonists, PPARβ/δ agonists, endotelial lipase inhibitors, Apo A-I mimetic peptides) and target the vessel wall (phospholipase A2 inhibitors) will provide incremental benefit for the high risk cardiovascular disease patient.
