Cytoreduktívna chirurgia (CRS) s hypertermickou intraperitoneálnou chemoterapiou (HIPEC) v liečbe karcinomatózy dutiny brušnej

Cieľ: Priblížiť problematiku liečby pokročilých malignít peritoneálnej dutiny použitím cytoreduktívnej chirurgie (CRS) v kombinácii s hypertermickou intraperitoneálnou chemoterapiou (HIPEC). Materiál a metodika: Autori približujú svoje skúsenosti s CRS a HIPEC na svojom pracovisku. Od mája 2010 operovali 13 pacientov. V 12 prípadoch bola použitá polootvorená (Coliseum) technika, v indikovaných prípadoch aj v kombinácii EPIC, v dvoch prípadoch bola použitá laparoskopia. Výsledky: Hodnota PCI bola v rozmedzí 0 – 39. Vzhľadom k malému počtu pacientov a rôznorodosti súboru je podrobnejšie hodnotenie ťažké. Bola zaznamenaná jedna vážna peroperačná komplikácia DIC. Záver: Vo vybranej skupine pacientov s pokročilou malignitou peritoneálneho povrchu, CRS v kombinácii s HIPEC predlžuje prežívanie.

Cytoreductive surgery and HIPEC in the treatment of peritoneal surface malignancies

Background: The mission of this article is to bring near the therapy of advanced malignancy of peritoneal surface by using cytoreductive surgery (CRS) combinated with intraperitoneal chemotherapy (HIPEC). Materials and methods: The authors presents their experiences with CRS and HIPEC at the own department. 13 patients underwent operation from May 2010. In 12 cases was used semi-automatic-closed (Coliseum) technic, in 2 cases combination with EPIC and in two case was used laparoscopy. Results: PCI was up 0 to 39 points. In respect of small file of patients and heterogenity of this file is further evaluation difficulty. It was registrated one serious peroperative complication - DIC. Conclusion: In the selected patients with advanced malignancy of peritoneal surface, CRS combinated with HIPEC improve surviving.
