Bipolární afektivní porucha u pacientky s karcinomem prsu jako komplikace protinádorové léčby

doc. MUDr. Ladislav Slováček, Ph.D., MUDr. Birgita Slováčková, Ph.D.

Autoři popisují případ rozvoje bipolární afektivní poruchy u 45leté pacientky s karcinomem prsu v souvislosti s aplikovanou neoadjuvantní chemoterapií dle protokolu AC-TH. Kazuistika poukazuje na potenciální riziko systémové chemoterapie včetně premedikace ve smyslu psychotropního nežádoucího účinku a nutnosti mezioborové spolupráce, konkrétně pak s psychiatrem. Časná symptomatická psychofarmakologická intervence umožnila bezproblémové dokončení neoadjuvantní chemoterapie s následnou parciální mastektomií a exenterací axily včetně adjuvantní léčby (radioterapie, bioterapie a hormonální terapie).

Kľúčové slová: karcinom prsu, chemoterapie, premedikace, psychotropní nežádoucí účinky.

Bipolar affective disorder in a female patient with breast cancer as a complication of anticancer therapy

The authors report on a case of chemotherapy-induced bipolar disorder after non-adjuvant chemotherapy AC-TH in a 45-year-old woman with breast carcinoma. This case report highlights the potential risk of chemotherapy and premedication causing bipolar disorder. At the same time it indicates the need to cooperate with the psychiatrist. Early symptomatic psychopharmacological intervention enabled the smooth completion of non-adjuvant chemotherapy followed by partial mastectomy and axilla exenteration including adjuvant therapy (radiation therapy, biotherapy, hormonal therapy).

Keywords: breast carcinoma, chemotherapy, psychotropic adverse side effect.
