Alternatívna a komplementárna medicína u onkologického pacienta

Používanie alternatívnej a komplementárnej liečby (KAM) u onkologických pacientov je pomerne rozšírené, čo nie je prekvapujúce vzhľadom na nedostatočné liečebné výsledky dosahované konvenčnými liečebnými postupmi. Nie všetky používané prostriedky KAM však boli relevantne testované v klinických štúdiách. Ďalší problém, ktorý je prítomný pri užívaní KAM, vyplýva z možných liečebných interakcií so súčasne používanou konvenčnou liečbou. Najdôležitejší faktor v prípade KAM je preto vymedzenie dostatočného času na diskusiu o možnostiach KAM s pacientom a jeho rodinnými príslušníkmi. Podkladom pre diskusiu je dostatočná informovanosť onkológa o KAM, ktoré sú v súčasnosti k dispozícii a väčšinou nadmerne medializované, bohužiaľ s nie vždy kompletnými údajmi o uskutočnených predklinických a klinických skúšaniach a ich možných nežiaducich účinkoch a interakciách s inými liečebnými postupmi. Článok podáva prehľad o v súčasnosti používanej KAM so zameraním na tie látky, ktoré boli klinicky testované, ale aj na tie, ktorých použitie je rozšírené, napriek nedostatočným alebo žiadnym výsledkom z klinického skúšania.

Alternative and complementary medicine in cancer patient

The use of alternative and complementary medicine (CAM) in cancer patients is widespread and it is not surprising as the results gained by conventional treatments are not sufficient. However, the results from the studies with CAM are not always sufficient according to their testing in appropriate clinical studies. Another problem that is present in the use of CAM is the possibility of drug-drug interactions between conventional therapies and CAM. Thus, it is of utmost importance that the oncologist possess a good knowledge of available CAM and provide a sufficient time for discussion with the patient and his/her family about possible alternative treatments and any downside risks. The cornerstone for pertinent discussion is sufficient knowledge on the part of the oncologist about those alternative treatments that are usually presented in the media with incomplete information about their relevant clinical tests and side effects. The following article presents a review of the current alternative treatment methods with a focus on the alternative drugs that have already been clinically tested, and secondarily on the alternative drugs that have been used even without sufficient testing in clinical trials.
