Endovascular treatment for critical limb ischaemia

Critical limb ischaemia is one of the most severe clinical manifestations of peripheral arterial disease of the lower limbs. It manifests itself by ischaemic rest pain, ischaemic ulcers, or gangrene. Even within one year of its onset, it is associated with a high mortality rate and the risk of loss of the affected limb. With each patient, the option of revascularization should be considered, with the main goal of preserving the limb and avoiding a major amputation. Technological advances of endovascular devices and introduction of new endovascular techniques have changed the strategy in revascularization therapy. This review summarizes contemporary evidence and capabilities of endovascular treatment such as percutaneous transluminal angioplasty, stenting and its various modifications, atherectomy, and recanalization of chronic total occlusions.
