Clinical application of a single-operator direct visualization system (SpyGlassTM DS) improves the diagnostic and therapeutic yield of endoscopic retrograde cholangiopancreatography
In many clinical situations differentiate benign from malignant biliary lesion is extremely difficult. Cholangiographic characteristics alone during endoscopic retrograde cholangiopancreatography (ERCP) or magnetic resonance cholangiopancreatography (MRCP) may not be specific enough to dictate management decisions of patients. The major diagnostic challenge represents indeterminate stenosis, in which the basic diagnostic procedures including transabdominal imaging techniques and ERCP with cytological brushing, are non-diagnostic. Peroral choledochoscopy can improve their diagnosis by visualization of the mucosa at the stricture site and obtaining directed biopsies in order to increase the diagnostic yield. Peroral choledochoscopy (SpyGlassTM DS) can improve their diagnosis by visualization of the mucosa at the stricture site and obtaining directed biopsies in order to increase the diagnostic yield. Except diagnostic utility the peroral cholangioscopy has significant application in the treatment of complicated choledocholithiasis.
