Včasná komplikácia neresekčnej liečby perforácie primárne nerozpoznaného Meckelovho divertikula
Early complication of nonresectional therapy of primary nonrecognized perforated Meckel´s diverticulum
Meckel´s diverticulum si the most frequent malformation of the gut. According post-mortem studies it´s found in approx. 2% of population. Complication rate is at about 4%. Most common complications are bleeding, obstruction, inflammation and perforation. The only suitable therapeutic procedure in case of complications is resection, what authors demonstrate on case report. Patient without previous surgery was admitted at the 2nd Clinic of Surgery FM CU with abdominal pain and physical and radiological signs of inflammatory acute abdomen. He underwent suture of perforation of small intestine and evacuation of interansal abscess by minimally invasive approach supposing alimentary origin of perforation. During early post operative period ileus occured. This was managed by partial resection of omentum and segmental resection of ileum including clearly visualized Meckel´s diverticulum with healed site of perforation by open approach. Limited view especially by marked inflammatory changes, and consequent unrecognition of malformation together with nonresectional surgery bring the risk of additional complications already in early postoperative period.
