Liver cysts - management and complications

Liver cysts are diagnosed more and more frequently due the increased use of diagnostic procedures for miscellaneous reasons. They are often found incidentally and mostly they are asymptomatic. Differential diagnosis is not always simple. Fenestration or „deroofing“ seems to be a procedure of choice in the management of simple cysts, which could be performed by miniinvasive approach. Beside approved efficacy and safety complications could occur, the solution of comprises challenging surgical procedures like high biliodigestive anastomosis or liver resections. This is demonstrated by authors in the case of female patient with previous cholecystectomy which has undergone laparoscopic deroofing of large symptomatic cyst with progressive growth. The biliary tree lesion has occured, which was managed by high hepaticojejunoanastomosis. Simultaneously there was a lately recognized communication between cyst and biliary tree. The ligation of left hepatic artery and left hepatic duct was used as an alternative to „almost consumed“ left liver lobe.
