Cellular angiofibroma of the small pelvis and perineum - case report

Introduction: Cellular angiofibroma is a rare benign mesenchymal soft tissue tumor. It typically appears in the vulvo-vaginal region in women and the inguino-scrotal region in men, but cases of its localization in the pelvic region, as well as extra-pelvic or retroperitoneal localization, have also been described. Case: In the case report, we present the case of a 57-year-old female patient with a cellular angiofibroma located presacral with spread to the perineum and right gluteal area, diagnosed by CT examination. 5 years ago, the patient underwent excision of a cellular angiofibroma from the area of the right labium, so it was a recurrent finding. During elective surgery, the tumor was completely extirpated from a combined laparotomy and perineal approach. The result of the definitive histological and immunohistochemical examination of the extirpated tumor tissue was a cellular angiofibroma, while the results were consistent in clinical correlation with the already known diagnosis of a cellular angiofibroma from an extirpated vulva tumor in the patient in the past. Conclusion: The treatment of cellular angiofibroma is surgical and its essence is the complete extirpation of the tumor, although even positive resection margins are not increased with the risk of local recurrence, which has been published in the world literature so far in only 2 cases.
