Psychiatrické příznaky u Parkinsonovy nemoci ve vyšším věku
Neuropsychiatric symptoms in Parkinson’s disease in older age
Neuropsychiatric symptoms are very common in Parkinson’s disease, especially in its later stage. They are often under-recognized and poorly managed. Early intervention may prevent progression of symptoms and development of life-threatening behaviour. Clinicians are advised to routinely screen for psychiatric symptoms in the office and modify Parkinson’s disease treatment accordingly or consider the addition of psychopharmacotherapy: rivastigmine in dementia and psychosis, SSRI or other antidepressive medication in anxiety and depression, an atypical antipsychotic (quetiapine, clozapine) in low doses in psychosis or aggressive behaviour if necessary. Nonpharmacological approaches are important. Case reports are presented.
