Submentálny absces ako komplikácia neliečeného atopického ekzému

Submental abscess as complication of untreated atopic eczema In the article the author describes the case history of a 21-month-old girl with a non-treated atopic eczema (short AE), whose health condition culminated in developing acute submental abscess after a non-treated acute infection of respiratory organs. Health problems of the child were intentionally overlooked in the family, especially by the mother who knowingly ignored any doctors’ or close relatives’ advice based on her own false considerations. The child was not vaccinated, the mother ordered the child dietetic restrictions regardless of the results of immunoallergic examination. The mother was not able to identify the urgency of the acute condition and the child was brought to hospital due to a coincidental meeting of the mother with a friend – ENT specialist. During hospitalization a dermo-respiratory syndrome was diagnosed and also treated. Complex medicamentous and surgical therapy was taken successfully with a fast and very positive effect. In the cultivation of the swab from the abscess it was found an infection of Streptococcus pyogenes and Staphylococcus aureus. Infectious complications with AE can become a reason for presence of different health states – from easy to life-threatening ones (abscess formation in subcutaneous tissues, in the brain, endocarditis formation, septic arthritis, osteomyelitis, sepsis and others). In 2018 consensual European guidelines for AE treatment in adults and children were published, among other things these recommend psychotherapeutic intervention in the family. At present we meet more often the parents rejecting any treatment of the children without any reason, similarly prevention of illnesses in the form of vaccination. An unsolved problem remains the legal scope of such a procedure threatening the health and life of the child taken by the parents.
