Use of chloral hydrate in pediatric home palliative care

Chloral hydrate is one of the oldest centrally acting sedatives and, despite its decline in standard clinical practice, it still holds a place in pediatric medicine. In pediatric home-based palliative care programs, it can be a suitable option in situations requiring symptomatic management of restlessness, refractory insomnia, dystonia, spasticity, or epileptic seizures. Its advantages include the possibility of enteral or rectal administration, a predictable effect, and a relatively low risk of respiratory depression compared to other sedatives. When appropriately indicated, chloral hydrate may represent a safe and effective medication for alleviating suffering during the endof- life period. This article outlines recommended dosing, associated risks, preventive measures, principles of parental education, and practical experience with its use in home settings.
