Case study of a patient with chronic obstructive pulmonary disease on non-invasive ventilation

Patients with exacerbated COPD and acute hypercapnic respiratory failure are at risk of repeated hospitalizations in a health care facility. We present the case study of a 66-year-old female patient with a diagnosis of COPD GOLD 4E with emphysema pulmonum, on bronchodilator and anti-inflammatory treatment with inhalation, ventilation NIV and oxygen therapy. Time analysis after 18 months captured the progression of the disease. The patient is provided with palliative care, requires assistance with all instrumental activities of daily living. Her mobility is significantly affected by shortness of breath. She uses a mobile or stationary oxygen concentrator at home. She is supplied with an oronasal mask for NIV using a positive pressure ventilator, to which she also connects oxygen during sitting and at the night NIV and oxygen therapy ensure the patient the highest possible quality of remaining life. Oral opioids were prescribed as symptomatic treatment for terminal COPD. The patient was offered hospice care, which she declined.
