Surgical management of primary liver cancer

Primary liver cancer is mostly composed of hepatocellular carcinoma and intrahepatal cholangiocarcinoma. The incidence of these tumours is rising worldwide, what may be caused by pandemics of obesity, high consumption of alcoholic beverages and in some countries also by high incidence of chronic hepatitis B and C. It is of utmost importance to emphasize prevention and screening plan for patients at risk with aim at early diagnoses and early therapeutic response. Surgical management forms the baseline of oncologic therapy in primary liver cancer and it is the best possibility for patient′s long term survival. In surgical management of hepatocellular carcinoma we use liver resection and for patients fulfilling rigorous inclusion criteria orthotopic liver transplantation. In surgical management of intrahepatal cholangiocarcinoma we use liver resection with adequate lymphadenectomy of hepatoduodenal ligament.
