Quo vadis, regionálna anestézia?

Konvenčné ako aj neurostimulačné techniky periférnych nervových blokád, ktoré sa vykonávajú bez vizuálnej kontroly sú vysoko závislé od povrchových anatomických orientačných bodov pri lokalizácii cieľového nervu. Tieto regionálne techniky majú až 20 % pravdepodobnosť zlyhania blokády. Viac pokusov a chýb pri hľadaní cieľového periférneho nervu môže viesť k iatrogénnemu poraneniu pacienta alebo traumatizovať pacienta bolesťou predovšetkým u pacientov s komplikovanými anatomickými pomermi. Prvé ultrazvukom navádzané nervové blokády boli opísané už v roku 1978. Ultrazvukom anestéziológ verifikuje umiestnenie nervových štruktúr a topografiu okolitých vaskulárnych, svalových, kostných a viscerálnych štruktúr. Poskytuje aj zobrazenie ihly, čím nám umožňuje jej bezpečné navedenie v správnom smere a hĺbke k nervu bez poranenia ostatných anatomických štruktúr. Možnosť sledovať a korigovať šírenie sa anestetika v reálnom čase počas blokády zvyšuje jej úspešnosť ako aj kvalitu. Zároveň sa tým znižuje množstvo podaného anestetika. Ultrazvukom navádzaná regionálna anestézia je cestou, ktorou sa v súčasnosti uberá moderná anestéziológia.

Conventional as well as nerve stimulator guided peripheral nerve blocks are performed without visual guidance, thus are highly dependent on surface anatomical landmarks. These regional techniques have a failure rate of up to 20%. In patients with difficult anatomy, multiple needle insertions can lead to iatrogenic injury as well as increased periprocedural pain. The first ultrasound guided nerve blocks have been described in 1978. With the help of ultrasound, the anesthesiologist visualises the location of nerves, vessels, bones , viscera as well as the course of the blocking needle, which enables him to insert the needle in the right direction and depth, avoiding injury to the adjacent tissue. The ability to see and influence the local anesthetic spread in real time makes the block more reliable and decreases the volume of local anesthetic. Ultrasound guided regional anesthesia is a modern state of the art anesthetic procedure.
